If a player has been banned for whatever reason, but games played by the player are submitted for rating, surely it is your responsibility, as Chairman of the FIDE Qualification Commission, not to accept those games for rating.

I doubt that the QC Chairman of the day, the late Mikko Markkula, was ever informed of the ban. I don't even recall seeing it in writing myself.

By the way, if it really was a two year ban then he should not have played in the 2006 Olympiad, which was 19 months later.

Other organizations have made arithmetical errors like that!
