Found at the online collections of the J. Paul Getty Museum, a series of manuscript pages (the Bonus Socius 'Ludwig XV' manuscripts collection) from the late 14th-century from Northern France, described as being from a "treatise on chess with 210 drawings and various games": 1, 2, 3 (text only), 4, 5, 6, 7.

That latter book looks to have all the answers (I was originally going to ask if anyone was able to decipher the diagrams).
Actually, that book only has some of the problems, so maybe some deciphering is still needed...
