I had a game many years ago where I offered my hand to my opponent because the game had ended in stalemate. He thought I'd resigned. The discrepancy only became apparent when we reported different results to the tournament controller. (It was sorted out amicably when my opponent thought about the final position for a minute and realised it had been stalemate.)
